For the Practitioner Profiles you can just email us: 1) your picture (see the bottom
of the submission form for the email link) 2) and the text for the main info....name, addr., phone, etc. 4)
and the categories (such as Specialities:, Memberships: etc.) with the text you'd like it to read.
You
do not need to design the ad - we will put it together at this end and email you a proof.
